Total kidney failure is not a nice way to live.  Dialysis takes hours, and between dialysis treatments the patient gets more and more ill from the buildup of metabolites in the bloodstream. The cost is staggering.  It is not impossible that Joe will be made penniless by the medical bills, even if he has health insurance - which he might not. (Go buy a few shows, help the old boy out!)
 
The alternative is, of course, certain death.  Or a kidney transplant.
